How the care works
Patients connect with a clinician through video visits and receive care in a familiar, comfortable setting. Visits are used to review symptoms, medications, and next steps together, so the plan stays current as circumstances change.

Why Home Doc
Patients often leave a hospital or rehabilitation stay with new prescriptions, specialist appointments, changing symptoms, and questions that do not fit neatly into the next scheduled office visit. Home Doc adds a responsive telehealth team during these high-need periods.
Some patients use Home Doc as a short-term bridge. Others benefit from ongoing psychiatric or palliative care. The plan depends on the patient's needs, eligibility, insurance, and clinician recommendations.

Not necessarily. Home Doc frequently works alongside an existing primary care physician, adding closer follow-up during a period of greater medical need. Some patients do choose Home Doc as their primary care practice. Intake can talk through which arrangement fits your situation.
Intake typically staffs and schedules a patient within twenty-four to forty-eight business hours, and patients are often seen within twenty-four to forty-eight hours after discharge when eligibility, records, and provider availability allow. Contacting intake before discharge usually shortens that timeline.
Visit frequency depends on the clinical situation, the service line, and what the patient's plan covers. During a transition after a hospital or rehabilitation stay, visits are often more frequent and then space out as the patient stabilizes.
Home Doc accepts plans from Medicare, Medicaid, Aetna, Cigna, and UnitedHealthcare. Not every plan offered by these insurers is accepted, so intake verifies your specific plan before care begins. See the Insurance page for details.
No. Home Doc does not provide emergency or urgent care. If you are experiencing a medical emergency, call 911. If you are in crisis or thinking about suicide, call or text 988.
