Clinician visits happen by telehealth across Colorado and Illinois.
Do not send urgent or emergency information by email. Call 911 for an emergency, or call or text 988 for a mental health crisis.
Choose a starting point
Patients, authorized representatives, and caregivers may contact Home Doc about Transitional Primary Care, Psychiatric Care, Palliative Care, or a situation that does not clearly fit one category. Intake can gather general information and explain which team should review the request.
Transitional Primary Care may be the starting point after a hospital or rehabilitation stay. Psychiatric Care may be appropriate for evaluation, medication management, therapy, mood symptoms, or cognitive concerns. Palliative Care adds support for symptoms, caregivers, and serious-illness decisions while other treatment may continue.
Prepare for intake
The inquiry process asks for a short set of details. Having them ready makes the first conversation faster.
Depending on the service, intake may also request discharge paperwork, a medication list, referring-provider information, legal-representative documentation, or relevant medical records. Coverage questions can be reviewed against accepted insurance before the first visit.
Submitting information starts a review; it does not create or guarantee an appointment. The team must consider service fit, eligibility, location, insurance, available records, and clinician availability before scheduling is confirmed.
Privacy and urgent needs
The patient inquiry form is the starting point Home Doc uses for new patients. Avoid sending urgent details or unnecessary protected health information through general email. If a caregiver is contacting Home Doc, the team may need patient consent or other authorization before discussing protected information.
Home Doc is not urgent care and does not provide emergency response. Call 911 for a medical emergency. For a mental health crisis, call or text 988 or call 911.

Use the patient inquiry form, or contact the intake team by phone at (833) 466-3302 or by email at intake@myhomedoc.com. A coordinator will follow up about service fit, insurance, records, and scheduling.
Intake asks for name, date of birth, insurance provider and policy number, requested service, phone, email, preferred contact method, and any comments about the situation.
A family member or caregiver may begin the conversation, but privacy authorization and patient consent may be required before protected information can be discussed.
Scheduling depends on intake completion, clinical fit, insurance verification, records, and provider availability. Submitting this form does not guarantee a specific appointment time.
