The first days after discharge
Why early follow-up after a hospital or rehabilitation stay matters.
Discharge often brings several changes at once. A patient may return to the community with new prescriptions, stopped medications, oxygen or equipment, therapy instructions, dietary changes, pending tests, specialist appointments, and home-health services. Even when written instructions are provided, it can be difficult to understand which item should happen first or who should answer a new question.
Transitional Primary Care creates a clinical checkpoint during that period. Home Doc typically aims to see patients within twenty-four to forty-eight hours after discharge when intake is complete and a clinician is available. The first visit helps identify immediate questions, review the care plan, and determine what should be coordinated next.
Appointment timing is not guaranteed. Intake completion, records, eligibility, insurance, and provider availability affect the first-visit timeline.
