About Home Doc

Built to make complex care feel more connected.

Home Doc began with a practical idea: patients should not have to navigate a vulnerable health transition alone while waiting for the next traditional appointment. It brings together clinical care, operations, and community relationships to provide consistent support through telehealth across Colorado and Illinois.

One coordinated care team. Clinical, operations, and facility-relations experience working together.

Enhancing care without replacing your doctor.

The team works alongside primary care providers, specialists, facility teams, home-health organizations, patients, and families. The goal is to keep information moving, respond to changing needs, and help each patient understand the next step in the plan.

The care model

A telehealth practice designed for periods of greater medical need.

Home Doc supports patients in Colorado and Illinois when frequent follow-up, medication review, psychiatric care, palliative support, or clearer coordination can make the care plan easier to manage. Clinicians meet with patients by telehealth. This allows the team to serve eligible patients across both states while staying connected to the people and organizations already involved in care.

Home Doc does not require every patient to follow the same timeline. A patient may need a short bridge after discharge, a longer course of psychiatric care, ongoing palliative support, or help coordinating until established follow-up is in place. Clinical recommendations, service fit, insurance, and availability shape the plan.

How collaboration works

Communication with the existing care team is part of the service.

When appropriate and authorized, Home Doc may coordinate with primary care providers, specialists, hospitals, rehabilitation teams, skilled-nursing or long-term-care facilities, home-health agencies, pharmacies, caregivers, and community partners. This can help surface gaps, reduce confusion about responsibilities, and keep relevant clinicians informed about important changes.

Home Doc is intended to add support rather than require a patient to leave an established clinician. If a patient does not have accessible primary care, intake can discuss whether Home Doc may be able to help and what information is needed to evaluate that role.

  • Patient and caregiver participation
  • Coordination with outside clinicians
  • Clear service boundaries
  • Telehealth access in Colorado and Illinois
  • Local Medical Assistant support where available

Where care is available

Clinician telehealth across both states, with limited local and facility support.

Physicians, physician assistants, and nurse practitioners provide clinician visits through telehealth to eligible patients located in Colorado or Illinois. Within twenty-five miles of Denver and Colorado Springs, Medical Assistants may be available to help with technology and collect in-person vitals when needed.

Home Doc may also provide in-person rounding at select participating long-term-care and rehabilitation facilities. These limited services are separate from clinician telehealth. Clinicians do not travel to a patient's residence; clinician visits happen by telehealth. See the full list of service areas.
